Case Manager- Health Care for Homeless Veterans (HCHV) Program

General Description: Our focus at CHHOP (Caring for the Hungry and Homeless of Peekskill) is housing and food equity. We operate a shelter for unhoused adults of all genders, including veterans, a collaborative housing and services support program for domestic violence survivors and their families, a permanent supportive housing program for chronically homeless individuals with a disability, a large ‘farm to pantry” food pantry and a rental arrears and community services program for immigrants and low-wage workers. CHHOP’S Veterans’ Case Manager is responsible for providing direct services to homeless veteran clients living transitionally at Jan Peek House.

Job Summary

The HCHV case manager is responsible for working with homeless veterans living at Jan Peek House to access permanent housing and support services that lead to independent living. The veterans case manager works closely with the VA and nonprofits providing services to veterans.

Responsibilities

  • Conduct timely intake and assessment of clients entering the program.
  • Link clients with medical, behavioral and other VA services.
  • Meet regularly with the VA Liaison.
  • Develop and coordinate service plans for clients, monitor progress and update plans as needed.
  • Link clients with appropriate providers and resources based on the needs and values of the client.
  • Assist clients with obtaining and submitting documentation required to obtain permanent housing.
  • Liaison with HUD Vash Case Managers.
  • Assist clients with interviewing for permanent housing.
  • Participate in formal and informal case conferences.
  • Maintain “audit ready” case files as required by governing body standards and CHHOP requirements.
  • Provide weekly updates to Supervisor and participate in Case Managers meetings.
  • Represent CHHOP at weekly Patriot Housing and social service provider meetings.
  • May require a flexible schedule.
  • May be asked to cover cases in multiple programs as applicable to organizational need.
  • Other duties as required.

Qualifications:

  • Education: BA/BS, Human Services field preferred.
  • Experience: Minimum of 2 years in human services preferred.
  • Knowledge of issues related to veterans, homelessness, and trauma.

Skills and Abilities:

  • Excellent oral and written skills.
  • Good time management skills, able to handle multiple tasks and assignments at the same time.
  • Ability to work with diverse populations.
  • Attentive to detail.
  • Self-starter, flexible.
  • Able to establish positive, therapeutic relationships with clients and maintain good relationships with community partners.
  • Proficient with Microsoft programs, data/case management systems.

Salary Range: $50,000 , medical and other benefits
